Safety

• Move about independently
• Require assist of only one caregiver
• Not abuse alcohol or drugs
• Not be a safety hazard to self or others
• Be capable of self preservation with minimal cues
• Manage medication on own or through assistance from staff
• Communicate his/her needs
• Take redirection from staff when wandering occurs
• Notify Park View staff when leaving facility

General

• Maintain a healthy lifestyle
• Take additional services when needed
• Pay amount in full when due
• Must cooperate with services and direction from staff
• Be reassessed for proper level of care or discharged if exceeding criteria

Socialization/Behavior

• Follow normal operation of facility without being disruptive
• Conform to socially acceptable standard
• Participate in Activities of Daily Living
• Manage one's own toileting needs
• Eat meals without assistance
• Manage incontinence independently
• Exhibit grooming and hygiene
